The Medical Examiner’s report gave new details on the death of Anthony Azarian, who passed away after the forklift he was operating fell seven stories down an elevator shaft.
The deadly incident happened at about 2:15 p.m. inside the CH Coakley Company near the corner of North 37th Street and West Wisconsin Avenue.
According to the report, Azarian was working for his father’s company, Azarian Wrecking, on the first day of a project in the building. Azarian was using a forklift with an attached bucket to remove bricks from the elevator shaft that was walled off.
The report says the bucket for Azarian’s forklift was “in a high position” before the forklift rocked, then fell over the edge of the shaft.
The forklift dropped 80 feet before striking the bottom.
Milwaukee Fire Department crews used the Jaws of Life and exricated him from the cage in his forklift, but could not revive him.

The Milwaukee Police Department tells WTMJ news partner TODAY’S TMJ4 the victim was Anthony Azarian, 32, from Racine.

The Milwaukee Police Department is investigating the death of a man who, reports say, fell several stories down an elevator shaft.

TODAY’S TMJ4 reports the man fell seven stories in the shaft, and was trapped in a forklift.
The only building that tall on the block houses a location for CH Coakley, a commercial warehousing and moving company.
